الصفحة اليمنى: جيت فيها المنصور ب. محفوظ الكوهين يطلق زوجته ست الدار بنت ابن نصر. بتاريخ الخميس 19 آب 1466 (= 1155 م) بالفسطاط. شهده مفوراك ب. ناثان هي-حافر وسليمان ها-لاوي ب. صموئيل. الظهر: تأكيد الاستلام، شهد عليه مفوراح ب. ناثان هي-حافر وسليمان ها-لاوي ب. صموئيل، مع ملاحظة مختصرة تصف الوثيقة. يوجد أيضًا على الجانب الأيسر ثلاثة أسطر بخط آخر تحتوي على مصطلحات تقويمية.
