Notes Pharmacopeia by Najīb al-Dīn Abū Ḥāmid Muḥammad ibn ‘Alī ibn ‘Umar al-Samarqandī (نجيب الدين أبو حامد محمد بن علي بن عمر السمرقندي, d. 1222). The main text is preceded by a table of contents (ff. 2v-3r). Begins (f. 3v, lines 11-16): هذا كتاب قراباذين نجيب الدين السمرقندي مؤلف كتاب الأسباب والعلامات وهو على ترتيب العلل والأمراض In the name of God, the Most Gracious, the Most Merciful, and in Him we seek help الحمد لله رب العالمين والصلوة والسلام على سيد المرسلين أما بعد فهذه أدوية مرتبة على بعض الأمراض من الأعضاء الرئيسة وغيرها أدوية علل الرأس حب القوقايا Ends (f. 90v, lines 4-13): ... وقد يسقى دانقًا من الذراريح مع دانق من العدس المقشر بماء فاتر بعد أن ينقع الذراريح في الرائب يومًا وليلةً ويبدل الرائب يفعل ثلاثة مرات فيكون كافيًا God willing Praise be to God, Lord of the worlds والصلوة والسلام على سيد المرسلين And his family and companions Everyone Amen
