ملاحظات	الصفحة اليمنى: جزء من كتابه ليشوع ب. صموئيل ب. يهوذا هو حافر (العريس) وست الأهل بات ييفت (العروس). شهده إيلي ها كوهين ب. حزقيال ها كوهين هي حافر، العازر ها كوهين ب. سليمان ها روش ب. يوسف هاصادق جاعون، إسحاق ب. طالب المرنم، حزقيال ب. ناثان، يهوذا ب. سليمان الكاتب، سعدية ب. إبراهيم، وجوزيف ها ليفي ب. يفيت، ويشهد له بيت دين. مؤرخ في 1[.]36 (= 1125م) بالفسطاط. على الجانب الأيسر: وثيقتان تخصان نفس الزوجين: هدية مقدمة للعروس من والدتها واتفاقية خطبة يتزوج فيها الزوج زوجته مرة أخرى بعد طلاقها.
