ملاحظات	الصفحة اليمنى: جزء من التوكيل الذي فيه إبراهيم ب. إسحاق يعين أفرايم ب. موسى الطبيب لتحصيل دين له على ييفت ب. ثابت بالفسطاط. بتاريخ الخميس 6 مارس 1368 (= 1056 م)، في الرملة. شهده حاييم هو الحفير ب. سليمان، جوشوا ب. إبراهيم، إسحاق ب. عزرا، يشوع ها كوهين ها بارناس ب. صدقة، سليمان ب. يعقوب، وبوعز المرنم ب. ديفيد. بعد توقيعات الشهود، تم التحقق من صحة التوقيعات الستة جميعها بشهادة من محكمة دانيال (ب. عزاريا)، ها ناسي، رئيس يشيفا فخر يعقوب. ويبدو أن التوكيل كتبه كاتب المحكمة، بينما الشهادة كتبها دانيال نفسه. الظهر: سطرين غير مرتبطين بالخط العربي.
