ملاحظات	الوجه: جزء من حرف عربي. الظهر: إيصال يتعلق بمزرعة الضرائب لأبي الحسن ب. وهب كتبه ميخائيل ب. عبد المسيح أمين الصندوق ومسجل لدى ديوان الحسابات نيابة عن ديوان الرقابة: أبو الحسن ب. وقد دفع وهب مبلغ الدراهم والنصف والثلث والثمن للعقارات بالفيوم، تحت إشراف القاضي دقة الملك مكين الدولة وأمينها، تلميذ أمير المؤمنين أبي الحسن علي بن محمد. بهار، والمحاسب أبو السري تيودور ب. يوهانيس. بتاريخ 25 ذو القعدة 403 هـ (= يونيو 1013 م).
