الصفحة اليمنى: جزء من عقد الإيجار الذي بموجبه سليمان هزاريز ب. يوافق حلفون على استئجار عقار في عسقلان من مؤسسة هيقديش (مؤسسة خيرية). تاريخ عقد الإيجار هو شهر أيار 488 [.] (= 1120-29 م). شهد صدقة ب. سعدية، مفوراح ب. سعدية، هارون ب. يوسف، ويعقوب ب. العازار. الظهر: سجل الإيجار السنوي المدفوع لشيوخ هقديش. الإيجار السنوي ربع دينار أو 8.5 دراهم. السجلات السنوية يشهد عليها الشيوخ ناثان ها كوهين ب. مفوراح ويفت ب. يشوع. أثناء سير السجلات ناثان ها كوهين ب. يموت مفورآه ويحل محله ابنه مفورآه ها كوهين ب. ناثان ها كوهين "القاضي الممتاز".
