Date of copying unknown, probably 1tth/17th century. Incomplete copy: concludes with chapter Fī al-adwiyah al-munbatah li-lahm ..., coming 16 leaves after the line corresponding to the end of juzʼ 12 of the 1955 Hyderabad edition. Copied in parts, each with its own colophon. Black leather binding, without flap. Fine, laid oriental paper. Written in an elegant Persian hand, 35 lines per page, in black and red ink, with catchwords on the verso of each leaf. Names of sources rubricated; chapter titles overlined in red. Some gaps left in text left for rubricated words that were never added. Laid in: a biography of the author (2 p.), and a letter addressed to Lady Osler from Dr. M. Sa'eed (2 leaves), dated 9 Mar., 1922, through whom the ms. was purchased in Teheran ; the envelope accompanying the aforementioned letter, reading notes in Persian, and an errata sheet. Incipit: باسم الله الرحمن الرحيم في السكتة و الفالج والحذر والرعشة. Three ownership statements and seals on f. 1a, one of which read: هو مما أنعم الله تعالى علينا في دار السلطنة إصفهان شهر رمضان المبارك سنة 1144 وكتب العبد الأقل ابن محمد سعيد الموسوي محمد باقر الموسوي.
